Nurse Practitioner role in Toronto's Long-Term Care Homes Division to autonomously diagnose, prescribe, and perform procedures for residents across three facilities. Four permanent full-time positions providing primary health care management of acute and chronic conditions for elderly residents.
Key Responsibilities
Diagnose, order and interpret diagnostic tests, and prescribe pharmaceuticals within legislated scope of practice
Deliver primary health care including management of acute/chronic conditions, health promotion, and disease prevention
Conduct focused health assessments, physical examinations, and interpret findings
Collaborate with Medical Director and management on clinical outcomes improvement and risk management
Maintain resident files and health records in compliance with applicable legislation
Qualifications
Current registration with College of Nurses of Ontario as Registered Nurse in Extended Class (Primary Health Care or Adult)
Experience as Nurse Practitioner in long-term care, acute care, rehabilitation, continuing care or primary care
Experience in long-term care with empathetic care for elderly and frail older adults
Certification in Geriatric Nursing from Canadian Nursing Association or equivalent
Valid and current Basic Life Support (B.L.S) certificate
