Key Responsibilities
Provide public health nursing services to individuals, families, groups and communities according to TPH policies and College of Nurses of Ontario standards
Conduct independent outreach, screening, health assessments and service provision in clinics, schools, shelters, daycares, workplaces and community locations
Complete home visits, assess health status and develop health goals; refer clients to TPH services and community resources
Deliver clinical interventions including STI/HIV testing, immunizations, pelvic examinations, Pap smears, venipuncture and emergency care
Collaborate with community agencies and schools to assess health needs, plan and deliver programs, and maintain electronic health records
Qualifications
Must be available to work varied shifts including days, afternoons and evening shifts
