Full-time temporary 11-month Public Health Nurse role with Toronto Public Health, delivering nursing services to families, schools, and communities across multiple Toronto locations. Reports to a manager within Community Health and Well-being.
Key Responsibilities
Provide public health nursing services to individuals, families, groups, schools, and communities according to TPH policies and College of Nurses of Ontario standards
Conduct health/service assessments and develop plans to meet identified needs at individual, family, school, and community levels
Assess and make referrals to other TPH services, health professionals, agencies, and community resources
Deliver health promotion and behavior change interventions based on assessed health needs
Collaborate with community groups, schools, centers, and agencies to assess needs, plan, deliver, and evaluate TPH programs
Qualifications
Bachelor's degree in Nursing (BScN) from recognized university, as recognized by College of Nurses of Ontario
Valid Registered Nurse (RN) licence from College of Nurses of Ontario
Experience in public health nursing or community nursing and providing health promotion services
Experience working with priority populations and ability to create safe, inclusive environments with diverse populations
Knowledge of population health approaches, client engagement principles, and health promotion theory
