Public Health Nurse providing support to Toronto Public Health program areas in strategic use of electronic medical records and health information systems, while delivering public health nursing services to individuals, families, groups, and communities across various settings.
Key Responsibilities
Maintain records and documentation according to TPH policies and College of Nurses standards
Provide public health nursing services in clinics, schools, homes, workplaces, and community locations
Conduct home visits, assessments, health education, counselling, and referrals for communicable diseases and family health services
Support management and staff in use of Profile EMR, Panorama, and iPHIS systems; develop training materials and provide orientation
Review lab reports for communicable diseases, follow up on test results, ensure treatment compliance, and conduct contact tracing
Qualifications
Bachelor's degree in nursing (BScN) from recognized university and valid College of Nurses of Ontario license
Experience using Profile Electronic Medical Records System for clinical service delivery
Considerable experience in public health or community nursing and with Public Health Sexual Health Clinics or Harm Reduction services
Considerable experience providing user support for health information systems (Profile EMR, iPHIS, Panorama)
Extensive experience developing training materials and providing staff orientation
