Key Responsibilities
Assess residents' rehabilitative needs through observation
Participate in multidisciplinary team meetings to develop and modify resident care plans
Develop and implement rehabilitation programs including ambulation, mobility exercises, positioning, and sensory stimulation
Conduct individual and group therapeutic programs and document progress on resident charts
Monitor residents' progress and adjust treatment based on observations
Qualifications
Completed post-secondary program in restorative care or equivalent education and experience
Considerable experience in Long-Term Care, Geriatric or Chronic Care setting providing rehabilitation programs
Experience conducting functional resident assessments and evaluating therapeutic programs
Fundamental knowledge of physiology, disease process, and disabilities related to elderly persons and young adults in long-term care
Verbal and written communication skills; ability to work cooperatively with staff and motivate residents
