Temporary full-time Social Worker position at Sunnyside Home providing psychosocial assessment, counselling, and admissions support to residents, clients, and families in a long-term care setting. Key Responsibilities
Provide psychosocial assessment, counselling, and develop/implement care goals with residents, families, and staff
Conduct pre- and post-admission support and facilitate smooth transitions into Sunnyside Home
Review and assess long-term care admissions applications and communicate status to applicants and LHIN
Provide social work consultation to multidisciplinary teams and intervene in crisis situations
Lead conflict prevention and resolution between families, residents, and staff; participate in care conferences and discharge planning
Qualifications
Master's of Social Work degree plus related experience in long-term care
Knowledge of psycho-geriatrics, cognitive impairment, psychosocial assessment, counselling, and community resources
Knowledge of and compliance with relevant legislation (privacy, health care consent, health and safety, public guardian)
Strong human relations, communication, and presentation skills; ability to mediate in sensitive situations
Acceptable Police Vulnerable Sector Check (Level 3); TB test and annual influenza immunization required
