Enclosed please find copies of the Tender Submission Form which includes Appendix E Breakdown Pricing; Appendix A List of Subcontractors; Appendix C Contractor Reference; Scope of Work; Preventative Maintenance Gas Fired Heating Equipment Work Detail Sheet; Public Tender General Conditions; and PHC Policy Statements. The Tender Submission Form and all Appendices must be submitted in the envelope provided with the required Bid Security in the amount of $2,000.00. Your company name and return address must be provided on the front of the envelope.
