ADDENDUM NO. 1

This addendum has been issued by the Town of Penetanguishene Public Works Department to revise and amend the Request for Quotation (RFQ) document. All information included within this addendum is to be incorporated within the RFQ package submitted by each contractor. (SEE ATTACHMENT)

Failure to include the information provided by this addendum within the submitted RFQ document will result in the disqualification of the contractor.

REMOVE AND REPLACE THE CORRESPONDING SECTIONS IN THE RFQ DOCUMENT WITH THE FOLLOWING:

The PHILIP H. JONES POLLUTION CONTROL PLANT PARKING LOT ASPHALT REPAIRS
RFQ document is to be removed and replaced in its entirety by the attached revised RFQ document.

• The item description for AREA 1 within the RFQ document has been revised.
